Progesterone Receptor and Ki-67 Labelling Index in Meningiomas-do they correlate with recurrence?
【摘要】:Context:Although most of the meningiomas are benign some of them are associated with less favorable clinical outcome.Major prognostic question regarding meningiomas is the prediction of recurrence which is largely dependent on histopathological type,grading,proliferation indices and progesterone receptor status.Aims:Our study was done to assess the expression of Ki-67 and progesterone receptor(PR) in different histological types of meningiomas and to assess if these correlate with recurrence rates.Methods and Material:Total 60 cases were collected for a period of 2.5 years,and all the tumours were clinically followed up for a period of 10years from the date of presentation..Haematoxylin and Eosin staining was done for histological typing and grading of the tumors.Immunohistochemical staining was done for Ki-67 and PR using standard immunoperoxidase technique and Ki-67 LI and PR score was calculated.Radiological assessments were done every year for the first 3 years,and then every alternate year for the next 8 years.Results:Mean Ki-67 LI was significantly higher in male versus female,in Grade II versus Grade I,in recurrent versus non-recurrent and in PR negative versus PR positive meningiomas.Similarly,PR positivity was significantly higher in female versus male,in Grade I versus Grade II and in non-recurrent versus recurrent meningiomas.Correlation between Ki-67 LI and PR expression was also evaluated and a strong inverse correlation was found between Ki-67 LI and PR expression.The clinical recurrence rate was found to be much higher in tumours with high Ki-67 LI,and although recurrence was higher in PR positive tumours the relationship with PR receptors did not reach significant levels.Conclusions:Our results establish that the immunodetection of Ki-67 LI and PR expression in meningiomas provides a practical tool for assessing the biological behavior of meningiomas.
